Testing Appointment Request Form Please fill out the provided form to request an appointment to take your proctored E-Learning exam. All appointments requested will need to go through an approval process before the actual appointment is granted. You will receive notification via email stating whether your appointment has been made or if further information is needed. DO NOT assume that you have an appointment UNTIL you have received a reply email with confirmation of the appointment. We will respond to your request within 1 business day to confirm or deny your appointment. If you are trying to schedule an appointment to be taken within the next 24 hours, please call our office at (907) 262-0227. Appointment requests via website are available only to students within the University of Alaska system. If you are attending a school outside of the UA system, please call our office directly at 907-262-0227 to schedule. * E-Learners and/or computer-based testers must schedule a testing appointment. * KPC Face-to-Face paper make-up testers may walk in during ANY posted proctoring hours.
